  • Rocky Moutain UFCW
  • Colorado Access
  • Child Health Plan Plus (CHP+)
  • Blue Cross & Blue Shield
  • Cigna
  • United HealthCare
  • Humana (MEDICAL ONLY) - accepting as a non-contracted provider, therefore co-pay fees are higher / out of network benefits. We do not accept EyeMed Vision or benefit for glasses of $300 through Humana.

ATTENTION:

Our practice is no longer an in network provider with VSP (Vision Service Plan) or EyeMed Vision. Your vision care is very important to us and we certainly want to continue with your family’s patient care.

WHAT IS A REFRACTION?

A refraction is part of your exam which determines your eyeglass prescription.  It involves questions such as, "which is clearer, #1 or #2" as different lens choices are shown to you. Vision insurance typically covers the exam and refraction. Medical insurance will not cover the refraction fee.

VISION VS. MEDICAL INSURANCE

Vision coverage (such as EyeMed) is mainly designed to determine prescription for glasses, help pay for eyeglasses or contact lenses and to evaluate the health of eyes.  It is NOT designed to deal with medical conditions, diagnose, and/or treatment plans.   When a medical diagnosis or condition is present (such as high blood pressure, glaucoma, macular degeneration, cataracts, scars, infections, to name just a few) it is necessary to file a claim for your visit with your major medical carrier and the co-pays, deductibles, etc for that insurance will apply as well as any non-covered service.  Vision insurance DOES NOT  cover medical eye problems, just as medical insurance DOES NOT cover routine vision problems.

OUR OFFICE DOES NOT MAKE THESE RULES, THEY ARE DEFINED BY THE INSURANCE CARRIER THEMSELVES.

Thank you for your  understanding as we are providing the best care for you.
